Passports
The Lac Qui Parle-Yellow Bank Watershed District is a designated Passport Acceptance Facility, under the jurisdiction of the U.S. Department of State, with authority to accept passport applications and respond to inquires.

Requirements
All applicants must appear in person! If under the age of 16, both parents must appear with minor.
For first-time applicants and all minors (<16), please bring the following:
- A Completed DS-11 Application
MUST use black ink, DO NOT sign until at the office.
DS-11 Application (Also available at the Passport Office) - Certified Copy of Birth Certificate
Government issued (not hospital) - Valid Drivers License (or other government ID)
If applicant is under the age of 16, both parents must provide a valid driver's license. - Passport Photo
From any photography studio or any select retail store. - Checkbook or Money Order
Fee payment to the US Dept. of State (Cash or Credit Cards not accepted due to federal regulations)
